Registration fee

Participantsī registration fees cover:

• Access to all Scientific Sessions
• Access to Exhibition Area
• Refreshments
• Dinner on September 17, 2007
  and/or dinner on September 19, 2007
• A set of Workshops Materials

Confirmation of registration

When your completed Registration Form together with payment or proof of payment is received by the Workshop Secretariat/Guarant, you will receive a written confirmation of your registration by fax or via e-mail: the Confirmation Letter. This Confirmation Letter has to be presented at the Registration Desk in order to receive the Workshops Materials and Conference Badge.

Please note the deadline of August 17, 2007 to benefit from the reduced rates. Registration Forms without payment cannot be processed. Registrations received after September 16, 2007 will be processed as on-site registration. Reservations for the dinner included in the registration fee must be indicated on the Registration Form. If fees have been transferred late and have not yet been credited to the account of the Workshop Secretariat/Guarant, a copy of the remittance order, certified by the bank, should be presented upon registration.

Cancellations and Refunds

The Workshop Secretariat/Guarant must be notified in writing about any cancellation of registration. The appropriate refunds will be made after the Conference. The following cancellation conditions will be applied:

Cancellations before September 3rd, 2007

full refund of the registration fee minus a handling fee of 30 EUR

Cancellations after September 3rd, 2007

no refund

Name change: If you should be prevented from attending, you will be given the opportunity to send a colleague in your place. Name changes should be notified in writing to the Workshop Secretariat/Guarant. A handling fee of 10 EUR will be charged for each name change.

Terms of Payment

Registration fees must be paid in EURO by:

Credit Cards

Only Mastercard/Eurocard, VISA, Diners Club and American Express credit cards are accepted.

Please enter your credit card number, expiry date and cardholderīs name in the appropriate space on the form. Do not forget to enter also the CVC2 (MasterCard/EuroCard) or CVV2 (Visa) code which is printed on the reverse side of your credit card on the signature panel, after the number of your credit card (last three digits). Please fill in the billing address for payments by American Express credit cards. The Credit Card Payment Authorization Form (yellow form) must be attached to your Registration Form.

According to the Czech law, credit cards will be charged in local currency – Czech crowns (CZK) by using the exchange rate of the Czech National Bank on the date of payment. The approximate exchange rate is 1 EURO=28 CZK (June 2007).

The participantīs name should be clearly legible, otherwise payment cannot be identified!

Note: All payments made by bank transfer have to be made net of all bank charges.

A copy of the bank transfer must be sent with the Hotel Accommodation Form.
